Patient Information

GI Alliance of Illinois accepts most major insurance plans. Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure.

All insurance claims are submitted to the patient’s insurance company. After 30 days, if no response is received from the patient’s insurance company, the balance will be billed to the patient. Patients will be required to pay on the day of service any co-pays that are listed on the patient’s ID card. Please check with your insurance company if you have any questions or concerns prior to your visit or procedure.

HMO patients are expected to obtain and bring their referral with them to their office visits. Payment will be expected at check-in if you do not have your referral.

If you do not have insurance coverage or if we are not part of your insurance network, you will be asked to pay for your visit at check-in.
